How Does Foster Care Case Management Work?
YFA partners with the Department of Children & Families (DCF) and Community Based Care lead agency providers to deliver professional child welfare case management services, including foster care case management. Our primary goals are to:
Advocate on behalf of at-risk children and families, both locally and with State and Federal legislators
Safely maintain or reunite children with their parents when possible
Ensure children's safety and stability with appropriate relatives or non-relative caregivers when reunification isn't possible
Build partnerships with organizations offering supportive services for parents and caregivers
Provide high-quality, seamless case management services
Ensure the Safety, Permanency and Well-Being of every child served
What Foster Care Services Are Provided?
Foster care case managers work with families and caregivers to coordinate comprehensive community services tailored to each child's unique situation, whether children are living at home with parents who need support, temporarily placed in out-of-home care, or requiring permanent placement when reunification isn't possible. Foster care services are provided to:
Keep families intact by strengthening and educating parents and caregivers
Reunify children who have been removed from their family
Maintain a stable, safe placement for children who are in out-of-home care
Locate appropriate, safe and permanent placement for children who cannot be returned to their parents or caregivers
What Does a Case Manager Do?
Case managers provide support for parents and caregivers, empowering them with the tools, services, and support needed to independently maintain a safe, nurturing environment for their children. Case managers evaluate each child’s specific situation, then connect parents and caregivers with specialized providers and services including:
Parent training programs
Childcare assistance
Housing services
Domestic violence support
Psychological and psychiatric care
Substance abuse and co-occurring disorder treatment
Family preservation services

All children have a fundamental right to a permanent, loving, and secure home that allows them to reach their potential.
The best place for a child to grow up is a safe and permanent home, preferably with family members.
All children are entitled to be free from any form of abuse (physical or emotional) by their parents or caregivers.
Siblings should be placed together both in relative placements and out-of-home care unless specific reasons necessitate their separation.
Seamless case management services should ensure high quality and maximum effectiveness for children and families.
Communities should provide support to families in creating a safe and nurturing environment for children.
Families actively involved in decision-making are more likely to develop the capacity to safely care for their children.
Services to children and families must be culturally competent, responsive, timely, and designed to improve and strengthen families.
The service delivery system must be held accountable to specific performance standards.
